Hypoactive sexual desire (HSDD)
PT-141 (Bremelanotide) is the first FDA-approved non-hormonal treatment for HSDD in premenopausal women (sold as Vyleesi). It activates melanocortin receptors in the brain — not vascular pathways — making it effective for both sexes. Unlike Viagra, it works on desire itself, not just blood flow.

Dosing too high initially — nausea is dose-dependent; 0.5–1 mg is sufficient for most users and significantly reduces GI side effects
Mistiming the injection — effects onset at 45–90 minutes; most users inject too close to the activity window
Expecting results when underlying hormonal deficiency is the issue — PT-141 needs baseline testosterone in normal range to produce consistent results in men
Antihypertensives — PT-141 causes a transient blood pressure increase; monitor in those with controlled hypertension
Nitrates — avoid; risk of cardiovascular event from combined vasodilatory/vasoconstrictive effects
Other melanocortin agonists (Melanotan II) — redundant; do not combine
PT-141 is the only approved non-hormonal treatment that targets sexual desire itself rather than blood flow. The nausea at onset is the most limiting side effect; eating something light 30 minutes before significantly reduces it. Start at 1 mg and titrate based on response — many users find 1–1.5 mg is the sweet spot with manageable side effects.
