Sermorelin is GHRH(1-29), the first 29 amino acids of endogenous GHRH. It was FDA-approved for pediatric GH deficiency and remains available through compounding pharmacies for adult GH deficiency. It produces a more physiological GH response than synthetic GH injections, and the pituitary retains its regulatory feedback mechanisms.

Expecting results as fast as synthetic HGH — Sermorelin works physiologically via the pituitary; results develop over 3–6 months
Injecting within 2 hours of a meal — must be fasted for adequate pituitary response and meaningful GH pulse
Stopping at the first sign of improvement — 6+ month cycles are standard for meaningful anti-aging benefit
Glucocorticoids — significantly blunt the GHRH response; avoid combining
Somatostatin analogs (octreotide) — directly block GH release; contraindicated with sermorelin
Thyroid medications — thyroid optimization before GH axis protocols significantly improves outcomes
Sermorelin is the most accessible legal GHRH in the US via compounding pharmacies. Results are slower and more physiological than synthetic GH injections — the pituitary retains its normal feedback mechanisms. Most prescribers combine it with Ipamorelin for the GHRH + GHRP synergy. IGF-1 monitoring at 6-week intervals is standard protocol.
